The UMR LiveWell rewards program begins in Oct. 1 of each year, and runs through Sept. 30. Participants (UMR account holder) can earn points for the following activities:
Employees are eligible to earn up to $500 ($200 on preventive care services and $300 on other activities) for completion of the below activities after you have opted in to the wellness program on umr.com:
- Annual Wellness Visit ($50)
- Breast Cancer Screening ($50)
- Cervical Cancer Screening ($50)
- Colorectal Cancer Screening ($50)
- Prostate Cancer Screening ($50)
- Cholesterol Screening ($50)
- Diabetes Screening ($50)
- Self Reported Flu Vaccine ($50)
- Action Plans ($50 points each – up to $250 maximum)
- UMR Challenges ($50 points each – up to $250 maximum)
- Ongoing Condition CARE Coaching Engagement (must complete three coaching calls to be considered engaged) ($100)
- Tobacco/Nicotine Cessation Coaching Completion ($150)
You may redeem your points for your choice of gift cards or merchandise from popular brands, stores and restaurants through Online Rewards (1 point = $1). Reward points may take up to 90 days to be available.
